Standard dropoff time is 8:30 AM
Standard pickup time is 12:30 PM (Morning sessions) | 5:30 PM (Afternoon sessions)
Early drop off is anytime between 7:30 and 8:30 AM
Late pickup is anytime between 5:30 PM and 6:30 PM

Please provide an emergency contact that can be used in situations where we cannot get ahold of the standard parents or any guardian authorized for pickup.
By checking the box below I understand that should a health problem arise and I or the emergency contact is unable to be reached, I consent to emergency medical treatment, which may include surgery, ambulance ride or other treatment as deemed necessary by medical personnel. I consent to the release of information for insurance purposes and guarantee payment in full for all required medical costs.
As part of the standard operations for the summer camp pictures or video may be taken of the participants that may be used in future promotional materials or other commercial purposes for Growth Chess and it's associated products. Growth Chess uses photographs, photographic images, names, and audio/video recordings of participants and guardians for general publicity in publications, on its website, on social media, in public relations, promotions, advertising, marketing and other commercial uses. Your presence in Growth Chess summer camp as well as off-campus activities constitutes your consent to capture and/or use your or your child's image or likeness without remuneration or compensation.
You have a right to opt out of inclusion in images/photographs and videos used for marketing, communications, and public relations purposes. If you do not opt out, you are aware that the image of you, your child, or associated guardians recorded on the Growth Chess premises or in Growth Chess related activities may be used by Growth Chess for the purposes stated above. If you wish to opt out, you must check the box below. This will remain in effect for the entire time your child is registered for the summer camp and will not pertain to other activities sponsored, organized, or ran by Growth Chess. You can opt back in at anytime through written notice
By checking the box below and submitting this form, the Undersigned, being a parent or guardian of the above named minor participant releases Growth Chess LLC and any associated subsidiaries or partners, henceforth referred to as "Growth Chess", its representatives, agents, servants, and employees from liability for any injury to said minor, resulting from any cause whatsoever occurring to said minor at any time while attending Growth Chess summer camp activities, including travel to and from camp and any field trips, excepting only injury or damage resulting from willful acts of such representatives, agents, servants and employees. Growth Chess is also released from any expenses resulting from the injury. Furthermore, you agree to indemnify and defend Growth Chess against all claims, causes of action, damages, judgements, costs, or expenses, including attorney fees and other litigation costs, which may in any way arise from you or your child's presence upon the facilities of Growth Chess.
Your child will have the opportunity to participate in various activities throughout camp that may contain their own level of risk. Growth Chess will provide trained staff who will conduct these activities when possible in the safest manner possible. You agree to hold Growth Chess harmless for any injury incurred during said activities and that participation in all activities is done freely and with intent by the minor.
Therefore, the submission of this form states unequivocally that the Undersigned consents and agrees to assume any and all risks associated with attendance of any and all activities hosted by Growth Chess
